How this website handles consultation requests.
Booking information
The request form collects the patient’s name, phone number, city or town, and a brief reason for consultation. Your reason may include health information. With your consent, these details are saved so the clinic can review your request and contact you. Please do not include medical reports, identification numbers, or detailed records.
Storage and access
Requests are stored in the clinic’s Cloudflare D1 database. They are not displayed publicly. The website also uses short-lived hashed network identifiers to limit repeated submissions. Booking details are not stored in your browser.

For questions about a request or to ask for correction or deletion, contact hello@anjucare.com. You may choose to contact the clinic directly instead of using this form.
About appointments
Submitting a request does not confirm an appointment. Availability, consultation fees, and connection details will be confirmed by the clinic. This website is not an emergency service.
